Will Insurance Cover My Visit?

Nutritional services are covered by many major health insurance companies.  However, each policy is different as to what they will cover.  It is your responsibility to contact your insurance company before our visit to inquire about coverage for nutritional counseling. 
I am happy to assist you with this process.

Important questions to ask your insurance carrier are:
1. Does my plan include nutrition counseling as a benefit? (the code for nutrition counseling is: 97802 )
2. Does it cover my specific diagnosis: gestational diabetes, diabetes, high cholesterol, etc? 
3. Do I need a referral?
4. How many visits are covered?

Cancellation Policy

Your appointment time is time that I set aside for you.  I would appreciate at least 24 hours notice for any cancellations or to reschedule your appointment.  Please call my office at (908) 889-5105 as soon as you are aware of any issue that will prevent you from coming to our scheduled appointment time.
